CREATED IN 2010 AS A TRANSPORTATION-BASED HOSPITAL DIVERSION PROGRAM, AHH'S BE THE CHANGE PROGRAM GREW ORGANICALLY AS IT COLLABORATED WITH OTHER AGENCIES TO ADDDRESS INDIVIDUAL AND SYSTEMIC CHALLENGES.
