Mission Statement

Service Coordination-set up and monitor home care services for the sick elderly and the disabled. Services include but not limited to providing caregiver assistance respite services set up structured day programming substance abuse counseling and mental health counseling. NYS Department of Health NHTD & TBI Medicaid Waiver Program We are a Service Coordination Agency who provides services to approximately 62 participants. We maintain daily notes set up and monitor all services for each participant which includes a monthly home visit. Also structured day programming ILST CIC HCSS and respite services have all been utilized while including other provider agencies who also provide services through The NYS DOH. Mission: Utilizing the NY state and TBI NHTD Medicaid waiver programs we provide hope to individuals with traumatic brain injury stroke or chronic debilitating medical conditions such as COPD Diabetes Alzheimers Heart disease.
