CCS COLLABORATED WITH CLIENTS TO HELP THEM DEVELOP INDEPENDENT LIVING PLANS; PROVIDING REFERRALS FOR SERVICES AND ADVOCACY WHERE APPROPRIATE, AND MONITORING PROGRESS. CCS MANAGED AND MONITORED RAPID REHOUSING/TRANSITION-IN-PLACE PROGRAMS FOR FAMILIES, DIRECT INTEGRATION INTO THE COMMUNITY AND PERMANENCY IN HOUSING. CHILDREN COMMUNITY SERVICES OFFERED A CONTINUUM OF CARE FOR ALL STAGES OF OUR CLIENTS LIVES, ULTIMATELY LEADING TO INDEPENDENCE. EFFECTIVE APRIL 2021, CCS CEASED OPERATION
