COMMUNITY HEALTH COLLABORATIVE, INC. CONSTITUTES THE COMMUNITY GOVERNING BOARD OF THEFEDERALLY QUALIFIED HEALTH CENTERS OPERATED BY UNITY HOSPITAL AND ROCHESTER GENERAL HOSPITAL. EACH OF THESE HEALTH CENTERS SERVES A MEDICALLY UNDERSERVED AREA OR POPULATION AND PROVIDES PREVENTATIVE AND PRIMARY HEALTH CARE SERVICES REGARDLESS OF THE PATIENT'S ABILITY TO PAY.
