QUALIFYING MEDICAL COSTS INCLUDE: PRESCRIPTION DRUGS, DOCTOR, DENTIST, MEDICAL LABORATORY TESTS, X-RAYS, MAMMOGRAMS, AND HOSPITAL BILLS. THE FUND WILL ALSO ASSIST WITH TRANSPORTATION EXPENSES (VIA GAS VOUCHERS AND FERRY TOLLS), MEDICAL SUPPLIES AND EQUIPMENT AS PRESCRIBED. DOLLAR LIMITS (PER INDIVIDUAL) ARE SET ON AN ANNUAL BASIS AND ALL FUNDS ARE PAID DIRECTLY TO PROVIDERS.
