The organization exists to provide palliative care for chronically sick individuals in the community it serves. The organization's palliative careactivities will be predominantly in-home or home setting support as defined by the Center to Advance Palliative Care and the National Hospiceand Palliative Care Organization. Palliative care will be 100 percent of the organization's activities, and these activities will be conducted by itsservice providers and/or volunteers.
