THE PURPOSE IS TO BECOME A SUSTAINABLE NETWORK WITH AN INTEGRATED SYSTEM WITH MULTIPLE SERVICES AND SUPPORTS THAT WILL DEVELOP NEW AND INNOVATED MODELS OF CARE, UTILIZE BEST PRACTICES, STABLIZE QUALITY OF SERVICE AND DEMONSTRATE EFFECTIVE AND EFFICIENT DELIVERY OF CARE FOR PEOPLE WITH MULTIPLE DISABILITIES INCLUDING PHYSICAL AND MENTAL DISABILITIES.
