DECREASING RECIDIVISM BY ADDRESSING THE SOCIAL DETERMINANTS OF THE HEALTH OF CITIZENS RETURNING FROM INCARCERATION. OPERATION GATEWAY HAS DEVELOPED A MODEL TO LEVERAGE THE LIVED EXPERIENCE OF CERTIFIED COMMUNITY HEALTH WORKERS, WHO ARE FORMERLY INCARCERATED AND ACTIVE IN ADDICITON RECOVERY, TO ASSIST IN NAVAGATING THE BARRIERS FACED WHEN REENTERING SOCIETY. OPERATION GATEWAY IS CURRENTLY AN ALL VOLUNTEER ORGANIZATION WITH DEEP ROOTS IN THE AFFECTED COMMUNITY.
