THE ORGANIZATION'S SKILLED CASE WORKERS, KNOWN AS SUPPORTS COORDINATORS WORK WITH THE INDIVIDUALS AND THEIR FAMILIES TO DEVELOP LIFE PLANS THAT ENCOMPASS LIVING AND WORKING ARRANGEMENTS, ONGOING SERVICE NEEDS AND ANY OTHER RESOURCES THAT WILL HELP THE INDIVIDUAL LEAD HEALTHY AND FULFILLING LIVES.
