(CCF) IS A COALITION OF MORE THAN TWENTY COMMUNITY-BASED AGENCIES SERVING OVER 15,000 MEDICAID-ENROLLED CHILDREN AND FAMILIES ACROSS NEW YORK CITY. CCF ADVANCES HEALTH EQUITY BY COORDINATING BEHAVIORAL HEALTH, DEVELOPMENTAL, AND MEDICAL SERVICES FOR CHILDREN WITH COMPLEX NEEDS, ENSURING THAT FAMILIES CAN ACCESS THE CARE AND SUPPORTS NECESSARY TO THRIVE IN THEIR COMMUNITIES.
